Philosophy and
Approach
My primary
focus is on the mental health needs and issues of adults, as well as
children of all ages. I believe that everyone is entitled to a
thorough assessment with open and honest feedback regarding the
findings. I employ a family-centered, eclectic counseling style
with cognitive and behavioral evidenced-based practices.
Code of Ethics
As a
registered Intern of the Oregon Board of Licensed Professional
Counselors and Therapists, I will abide by its Code of Ethics. I
am under the ongoing supervision of Cherry Hartman, LCSW.
Education
I Hold a
Master of Arts Degree in Counseling from Trevecca Nazarene
University in Nashville, Tennessee. Course work and experience
included: Lifespan Development, and Family and Grief Counseling.
Additional trainings have included Dialectical Behavior Therapy and
Play Therapy. I have had extensive experience in crisis
intervention counseling and services. I am a Licensed Qualified
Mental Health Professional in the State of Oregon. I have had
the privilege of providing mental health services for adults,
children and families in a variety of settings for approximately 18
years.
Continuing
Education
To maintain my license, I am
required by the state to participate in annual continuing education,
taking classes in the subjects relevant to this profession. I may
substitute professional supervision for part of this requirement. I
will be happy to explain what that entails and how it relates to you
as my client.
Fee Schedule
Fees for counseling services are
$105.00 per hour of counseling. An adjustment in fees is possible in
cases of need and when discussed in advance with the counselor.
Client Bill of
Rights
As a client of an Oregon licensee,
you have the following rights:
1. To expect that a licensee has
met the minimal qualifications of training and experience required
by state law;
2. To examine public records maintained by the Board and to have
the Board confirm credentials of a licensee;
3. To obtain a copy of the Code of Ethics;
4. To report complaints to the Board;
5. To be informed of the cost of professional services before
receiving the services;
6.
To be assured of privacy and confidentiality while receiving
services as defined by rule and law, including the following
exceptions:
a) reporting suspected child
abuse
b) reporting imminent danger to client or others
c) reporting information required in court proceedings or by
client’s insurance company, or other relevant agencies
d) providing information concerning licensee case consultation
or supervision
e) defending claims brought by client against licensee.;
7. To be free from being the object of discrimination on the
basis of race, religion, gender, or other unlawful category while
receiving services.
